Athletic Clearance Instructions
IMPORTANT: PLEASE DO NOT EMAIL OR HAND IN ATHLETIC FORMS. ALL FORMS MUST BE COMPLETED AND UPLOADED THROUGH THE ATHLETIC CLEARANCE WEBSITE.
Step 1: Download & Complete EL2 Physical Form.
- Must be signed by your child's physician
- Only page 4 is required for upload
- Valid for 1 year from the exam date (e.g., May 22, 2025 - May 22, 2026)
Step 2: Complete Safety Courses (NFHS/FHSAA)
- Students must complete all of the following free online courses:
- NFHS Heat Illness Prevention
- NFHS Sudden Cardiac Arrest
- FHS Concussion in Sports
- FHS Sportsmanship
Certificates do not need to be uploaded, but course completion is required.
Step 3: Register & Submit Online
Go to: https://athleticclearance.fhsaahome.org
- Click "Create Account" and follow the prompts
- Verify your email, then click "New Clearance"
- Choose:
- School: Riviera Prep (Miami)
- Year: 2025-2026
- Sport(s): You may select multiple - Complete required sections:
- Student Info (include 7-digit Riviera ID from FACTS)
- Parent Info
- Medical Questions
- Parent & Student E-Signatures - Uploads:
- Page 4 of EL2 Physical Form (Only upload page 4 of the EL2. A clear photo from your phone is acceptable.)
