Athletic Clearance Instructions

IMPORTANT: PLEASE DO NOT EMAIL OR HAND IN ATHLETIC FORMS. ALL FORMS MUST BE COMPLETED AND UPLOADED THROUGH THE ATHLETIC CLEARANCE WEBSITE.
 

Step 1: Download & Complete Required FHSAA Forms

EL2 – Physical Evaluation Form

  • All pages must be signed and dated by your child's physician on the day of the examination.
  • Only page 4 is required for upload
  • Valid for 1 year from the exam date (Example: May 22, 2026 – May 22, 2027)

EL3 – Consent and Release from Liability Certificate

  • FHSAA has updated this form for the 2026–2027 school year
  • Parents and students must complete the newest version

NEW: EL1 – ECG Screening Form
Beginning July 1, 2026, the EL1 ECG Screening Form is required for:

  • All incoming 9th grade student-athletes
  • Any student in grades 10–12 participating in an FHSAA sport for the first time

Important notes:

  • An ECG completed on or after July 1, 2024, will satisfy the requirement
  • Returning student-athletes who have previously participated in FHSAA sports are not required to complete an ECG, but it is strongly encouraged

Additional Forms (If Applicable)

  • EL1/2s – Required only if a medical professional requests follow-up evaluation related to the EL1 or EL2
  • ME1 – Completed only by a licensed medical doctor to exempt a student-athlete from the ECG requirement

Parents may obtain ECG screenings through their healthcare provider or through local opportunities offered by Who We Play For.


Step 2: Complete Required Safety Courses (NFHS/FHSAA)

Students must complete all the following free online courses:

  • NFHS Heat Illness Prevention
  • NFHS Sudden Cardiac Arrest
  • FHSAA Concussion in Sports
  • FHSAA Sportsmanship

Certificates do not need to be uploaded, but course completion is required.


Step 3: Register & Submit Online

Go to: Athletic Clearance Website

  • Click “Create Account” and follow the prompts
  • Verify your email, then click “New Clearance”

Select:

  • School: Riviera Prep (Miami)
  • Year: 2026–2027
  • Sport(s): You may select multiple options

Complete all required sections:

  • Student Information
    (Include the student’s 7-digit Riviera ID from FACTS)
  • Parent Information
  • Medical Questions
  • Parent & Student E-Signatures

Uploads:

  • Page 4 of the EL2 Physical Form signed and dated by the physician
    (A clear photo from your phone is acceptable.)
  • EL1 ECG Screening Form, if required
