Athletic clearance

Ready to play? Complete your athletic clearance online before participating in an FHSAA sport.

All athletic forms and certificates must be completed and uploaded through the Athletic Clearance website. Please do not email or hand in forms to the school.

What you’ll need -

Complete the required FHSAA forms

 

Additional forms, if applicable

  • EL1/2s – Required only if a medical professional requests a follow-up evaluation related to the EL1 or EL2.
  • ME1 – Completed only by a licensed medical doctor to exempt a student-athlete from the ECG requirement.

Parents can obtain ECG screenings through their healthcare provider or through local opportunities offered by Who We Play For.

Complete the required FHSAA forms

Complete the required safety courses

Students must complete four free NFHS/FHSAA courses covering:

  • Heat illness prevention
  • Sudden cardiac arrest
  • Concussion in sports
  • Sportsmanship

Upload each certificate as you complete the courses.

Submit your clearance online

Create an account on the Athletic Clearance website and select: Riviera Prep (Miami) | 2026–2027 | Your sport(s)

You’ll need your student’s 7-digit Riviera ID from FACTS, along with parent and student information, medical responses and electronic signatures.

Please upload the following documents through the Athletic Clearance website:

  • Page 4 of the EL2 Physical Form, signed and dated by the physician A clear photo from your phone is acceptable.
  • EL1 ECG Screening Form, if required
  • Safety course certificates, as you complete the courses

Being a student-athlete at Riviera has taught me how to lead, mentor younger students, and set an example for others.

I chose Riviera because the school genuinely cares about its students and makes a conscious effort to help us grow into well-rounded individuals.
